Client Registration
Please be aware that our registration process differs from that of other prenatal providers because we need to ensure each client is within the risk criteria set for us by Illinois state and CABC regulations. This means we need to review each client before accepting them into care, which makes our registration process lengthier than other experiences you may have had. We are constantly working to ensure the process is as simple and efficient as possible for you and us. We are grateful for your patience and understanding during this process. These conditions fall outside the established eligibility criteria for birth center care due to the associated level of risk. Please note this list is not all-inclusive and risk assessment is ongoing throughout the pregnancy:
Pre-pregnancy BMI <18 or >40

Chronic high blood pressure
Diabetes, Type 1 or Type 2
HIV positive
Thalassemia
Homozygous Factor 5
Sickle cell disease
Systemic Lupus
Antiphospholipid
Substance abuse disorder
Current pregnancy with multiple gestation (Twins)
History of manual removal of placenta
History of uterine wall surgery
ADDITIONAL EXCLUSION CRITERIA IF VBAC (Vaginal Birth after C-section):
History of two or more prior c-sections
Pregnancy interval of <19 months from previous c-section to current pregnancy due date
Classical incision (Vertical up and down on abdomen and uterine c-section scar)
*If none of these apply, please proceed to below registration form. If transferring care mid-pregnancy, additional risk criteria apply and will be reviewed along with your prenatal records. Please note, inadequate prenatal care may make birth center care no longer appropriate or safe. (Criteria updated per IL Regs and Rules-8/25/26)
